County Planning Formats?

Can someone point me in the direction of where to find planning formats which different counties have created in line with the new EYFS?? I am already aware of Essex and Durham, but I am sure I read somewhere on here of others and of course cannot seem to find them again.

Sorry don't know of any more.. but what a good idea to put links to all those we know in one thread.. makes life so much easier when searching..
