Forms For Baby Room

Need to make up a form to go through with parents when they drop off their babies,i dont want to leave anything important off.Does anyone know of any website i could get a template of sorts,(all i have found are American)that i could adapt to my needs.So much to think of and we open on monday so all these need to be done.Just be told i have to do them and need a little advice .Is there anything esle that i need ?

I dont know a web site, but the forms I've seen have a space for feeds, drinks, sleeps, nappy changes, how much, what time etc and a box for comments about their day i.e. grumpy, happy, teething, new foods tried. Theres also a box which says what activities were readily available and the kind of things the baby enjoyed using.

Some places get the parent to sign and then give them a photocopy, especially valuable if the child is picked up by someone else.

Have found some of these thanks its the form that you would fill in with parents asking about babies rountines so we can know as much about their child to keep to their own rountine so should include .how much milk they drink how long do they sleep in what position ,have their had they injections etc.
