Ideas Needed For Parent Questionnaire On Induction Home Visit Program

Hi

As part of my PM I have decided to find out our parents views on the Induction process and Home visits in school.

This year we seemed to have many more parents who wanted their child to start full time immediately not over a four / five week period.

Has any one already done this before? I'm hoping that it will save me some time and give me some ideas on how to go about it.
