Learning Journeys-parent/family Comment Form

Hello, I have done a forum search but cannot find what i am looking for. I am hoping to see some examples of comment forms that you would include as part of the learning journey for parents and other family members to write comments in. My mind is a blank at the mo! perhaps something that could be combined with WOW vouchers? thanks in advance

My parents just jot comments down on slips of paper and I attach these to the pages in the file

Same as Cait, parents put their comments onto post it notes.

My parents take their children's folders home - usually once every six weeks or so - they then add their comments.......we actually write - Records Home (date) Parents Comments:

 

This works really well for us - parents always write something
