Consent Form

Do you have other consents in place, i.e. emergency consent, consent to going by minibus, will you take them with you? - and would a parent instinctively know of "anything else they need to tell you", could you give them a hint at what you might feel is relevant, i.e. allergies to animal fur etc., so no "petting", fears. I have always been totally afraid of birds so being encouraged to go anywhere near birds would have absolutely freaked me as a child!

 

Given farm visits in recent years and e coli outbreaks, you say you are going to risk assess the visit beforehand, will parents be given a copy of this or at least a breakdown of what you will be doing to minimise risks?

 

Why do we take them on visits? It's a nightmare.

Have to say that all of our trips are open to parents and we seldom have more than 4 children who are unaccompanied.

 

Depending on the time of year we normally have 4/5 staff attending so the ratios are high.

 

We have a risk assessment for where we are visiting and share this with the parents in case they need to split up, do different things and the consent form acknowledges that if parents are attending then they are in charge of their children (unless of course they request extra help ie they have a younger sibling or a child with SEN.)

 

We have had some super family trips!
