Guided reading record sheets

Starting guided reading this half term, was wondering if anybody would share their recording sheets for ideas?

We used the ones on Lancs website which were freely available but I think this has changed now. There were other others out there though. I used some to devise a simple plan.

Will find you mine over the weekend if I don't post them give me a nudge. as I'm too tired to search now at 3.30 am when I've just changed everything I'm doing again courtesy of my SLT who have not one shredof EYFS experience between them!!
