Guest Biker Posted March 10, 2013 Posted March 10, 2013 I wonder if any of you clever peeps can help! We are a preschool attached to a main school most of our children feed into the school. We currently form our baselne assessment through obs on the children but as our intake is staggered to ensure we can support our children to settle well it can be after half term before we manage to collate information for the whole group. We used to send out a questionaire to parents to aid our planning for individual needs and would like to revive this to use as initial baseline . Does anyone use a questionaire in this way and if so would you be willing to share an example? Thanks Biker.

AnonyMouse_1027 Posted March 10, 2013 Posted March 10, 2013 we have an 'all about me' form and as well as the usual questions have things like: how many words does your child say? does he make friends easily? can he climb? simple questions but gets the areas of learning needed for a baseline
