Registration form - what is on yours?

My registration form seems to get longer and longer and has on it:

Child's Name, Date of Birth, Ethnicity, Religion, Address, Preferred name, gender, first language

Parent 1 and Parent 2 Names, addresses, phone numbers (work, home and mobile), Y/N to parental responsibility

Emergency contacts 1, 2 and 3, addresses, phone numbers (work, home and mobile)

Sample signatures and passport size photos of parents and emergency contacts plus email addresses for parents

GP name and address and phone plus health visitor info

Immunisations

NHS number

Allergies / Medical Conditions

CAF details if any

SEN details if any

Social Care involvement if any

Other setting information e.g., Childminder, Children's Center or other nursery provision

Other agency involvement e.g., SALT, Paediatrician

Info about 2 year progress check e.g., has it been done and if so by whom

Permissions for things like teeth cleaning

Most importantly declaration by parents agreeing to terms and conditions etc.

wow sue -your form must be about three pages long lol .

we also ask parents to consent to any emergency medical treatment, including blood to be given to their child / photographs being taken of their child / staff to support children to apply sun-cream (children can do an all day so may need it re-applied after lunch) and give permission for the pre-school to contact any persons named on the form if we have any concerns about their child ,including any health care professionals x

Can I ask is the NHS number in addition or instead of a Nat Ins number? Desperately trying to keep it all to 2 pages...not looking likely!

A national insurance number is required if applying for 2 year funding under the current forms criteria.

But I don't ask for them on our own registration form.

Our registration form purely has child's name, DOB, parents/ carers names, address, telephone numbers including mobile.

Have they registered anywhere else.

Preferred days.

Have they had an introductory visit.

Any other comments section.

Some parents register online, then book a visit.

Some book a visit then register their child.

 

We then have a seperate personal details form, which includes.

Child's name, preferred name, DOB, birth certificate number, address, telephone numbers, home, mobiles, doctors number, address, telephone number, collection persons authorised, telephone numbers, home, mobile , work telephone numbers/ emergency contact numbers, health visitors name, social worker, immunisations including seeing the red book, allergies, religion, language spoken at home, any concerns regarding health and educational development, drink preferences, toileting needs, including nappies, pull-ups, potty, toilet, stand sit, words used when needing to go.

Then finally

I agree to the terms and conditions as detailed in our prospectus and signed by them, and by the /Supervisor.

 

We then have a seperate consent form, which includes, leaving the premises in a medical emergency, consent for medical treatment including any exceptions, observing, photographing / video, journals, multi agency links, medical conditions requiring possible administration of meds, inhalers, epi-pens etc,

They then complete our meds book on the child starting.

Leaving the premises for short outings,

Transition consent.

Phew sure I've missed something.:)

We have updated ours to include NI on free for 2 forms but we also ask the question about comforters and cuddly toys so the staff know what the child requires if they are upset or unsettled. we also ask for a photograph for their wall peg and a series of small family photographs so we can make a tagged set so the child can see loved family members if they become upset or distressed.

medication or routine also helps us settle the child in quickly and reassures parents we care about each individual, how about asking some of the parents what they expect to see on the form, you may be surprised by their replies.

Come on Sunnyday get with the programme! - Not National Insurance (NI) Number but National Health Service (NHS) Number ;)

These are quite different -the NHS number is sometimes needed to access medical care and services (and sadly we have needed it in the past to "get past" a GP's receptionist)

I haven't yet needed NI numbers - although perhaps if they issued them to children we could tap into their benefits to help make up for the funding shortfalls xDxD

We ask for parents NI number for entitlement to funding purposes/ID check and allegedly it helps track down non paying parents

Never asked for NHS number

Just looked up NI number use and it appears it shouldn't be disclosed willy nilly - we inherited the child entry record and tbh never looked in to it. Will look at reviewing it now!

Well I kinda thought that - hence my surprise - but then it seems from reading other posts that it is required for 2 year old funding - so I don't know now - anyways doesn't affect me either way! :ph34r:xDxDxD
