Our one at the moment doesn't really tell me much and I want to make it useful rather than just a box ticking exercise thanks in advance!. I do a tally chart of the type of accident and area of accidents over a half term period. Then I write a statement at bottom I.e physical area increased accidents occurred Etc etc etc' then if any action required, this statement is then shared at staff mtg. x Quote Link to comment Share on other sites More sharing options...

We had a complaint regarding an accident a while back and this is something that they asked for evidence of and wanted to know how often we undertook them...
