I have previously used a staff declaration form from here for staff to sign relating to their dbs status, called suitable persons check.......for the love I cannot find it has anyone got a copy they could send

if you google early years staff declaration lots of examples come up - the one from wigan council looks good - sorry don't know how to do a link!!
