I work in a foundation stage setting and I am trying to find some kind of booklet that I can use to gather information from the child's family. Rather than reinvent the wheel I thought someone out there may be able to help. The kind of thing I am looking for would allow the parent/carer and child to tell the setting about the childs likes and dislikes, any prior learning such as number recognition, colour recognition, prefered hand, nursery rhymes they like etc. If anyone has anything like this they would be willing to share I would be really grateful.
