Guest Wolfie Posted May 3, 2007 Posted May 3, 2007 I'm sure this has been discussed before but I can't find anything when I search! Has anyone got a prompt sheet or list of headings that they use when chatting to parents and gathering initial information about a baby/child starting nursery? I know that the regsitration from contains all the necessary things like contact details, medical history and details, etc. etc - what I'm looking for is the "extra" bits that are going to help the staff make the child feel secure and happy.......I think I've got everything but there's always something that you haven't thought of that would make a difference! Thank you!
AnonyMouse_64 Posted May 3, 2007 Posted May 3, 2007 This is the form we get parents to fill in. Initial_Child_Profile.doc
Guest Posted May 3, 2007 Posted May 3, 2007 I have attached our form-we also have a personal record form with the details you have already mentioned Wolfie. Might as well attach that as well. So these are the two forms our parents complete. Linda All_about_me_1.doc All_about_me_2.doc personal_information_rcords.doc Parent_info_Personal_Records_2.doc
Guest Wolfie Posted May 3, 2007 Posted May 3, 2007 Thanks for that Linda - very useful! I really like your little picture, it's great!

Guest Wolfie Posted May 5, 2007 Posted May 5, 2007 Thank you for that too - it's all coming together now!
Guest Posted May 5, 2007 Posted May 5, 2007 Lots of good examples attached but thought I'd add mine as I'm always downloading other people's stuff! entry profile is used on entry to our school nursery at 3. Little windmills profile is used for entry to our private provision at 2 years. entry_profile.doc little_windmills_entry_profile.doc
Guest Posted May 5, 2007 Posted May 5, 2007 Not long before I left my last nursery I changed the forms so that we also asked about the occupations and hobbies/skills of parents and carers, and whether we would be able to utilise them in nursery from time to time.
Guest Wolfie Posted May 5, 2007 Posted May 5, 2007 That's a nice format nsunshine - what a creative lot we are!
